Coach Forms and Requirements

The following forms are required for coaches:

Coaches are required to watch the Michigan Department of Community Health video entitled, “Youth Sports Training” - CDC 'Heads Up' Concussion In Youth Sports" prior to coaching. The link is http://www.michigan.gov/mdch/0,4612,7-132-54783_63943---,00.html.  Coaches need to print the certificate of completion and submit to the athletic director.

Coaches must also complete a Protecting God's Children Workshop prior to coaching.  Registration can be found at https://www.virtusonline.org.
